Calendar control/copying form fields

I am using Office 2004 on this MacBook. I want to create a form in Word. The form will consist of a list of future events, showing the date on which each is to occur.

The form is for use by individuals that want to provide details of events that they will be organising, so they must be free to enter whatever description they choose.

Ideally, I should like to be able to insert a control that would display a drop-down calendar to enable users to select a date from it. However, the only such calendar generally available appears to be an ActiveX control, which rules it out for this note-book. There is Win/Mac Scalable Calendar Control available on-line, but the technicalities have for the time being defeated me. Question 1: Can any kind person explain in simple terms what the steps are ?

As an alternative, I have been considering using combo-boxes from which users could select day, month and year.

Question 2: Is it possible to copy-and-paste them to where I want them to appear ?

I should say that I have (strictly amateur) experience of simple programming in QBasic and in Visual Basic, but am distinctly out-of-practice with both, so am hoping to avoid anything too intricate.

I should be grateful for any help !

Ignore Question 2 -- I have got the answer to that. Unfortunately, I have at the same time learned that I cannot use combo-boxes for the purpose, as there is a 25-entry limit for each list, which rules out a list of days of the month.

But is Question 1 really such a hard question ? Perhaps naively, I supposed that many people would be creating MS Word forms with a drop-down date-selector, indeed that I was pretty well the only person using forms that was still not sure how to do that.
